The health check results booklet was created to support health professionals when carrying out a health check. As results were collected during an examination patients often expressed an interest in taking this information away with them. As nurses were initially writing this down – it was felt this was a great opportunity to provide the requested information along with some health advice covering the major contributing factors to heart disease and poor health.
